Workshop Attendees

If you attended a Soulographer Workshop (not a Business Bootcamp or Soul Sistas Workshop) in 2007, 2008 or 2009, please email me the following information so you can be mailed your DVD:

- Your Name
- Your current address & Phone
- Workshop you attended
- Payment method you used (either Paypal or Credit Card)

Note: You MUST email me at the correct email below otherwise you will not be sent your DVD.

You must get your info to me within 30 days of the DVD's release or you forfeit your complimentary DVD.
